    99(2) How to Enter.

    103(a) To enter a voucher number into the drawing, players must enter on the 117Florida 118Lottery’s website at 121flalottery.com122. On the home page of the 129Florida 130Lottery’s website, players can click on the CASH4LIFE Bonus Bucks Promotion banner(s) 142and follow the directions.

    146(b) The entry voucher will be attached to the bottom of the 158CASH4LIFE 159ticket. The voucher entry number is located at the bottom on the front of the voucher. Players are to enter the first 13-digits of the 19-digit voucher entry number. Qualifying 189CASH4LIFE 190ticket purchases will produce entry vouchers from the beginning of the promotion period until 20410:59 205p.m. 206(207ET208) 209on 210February 28, 2122012138214. 215A 216player will be able to enter his or her voucher numbers beginning at the top of the hour after the 236CASH4LIFE 237ticket is purchased. 240A player entering a voucher number prior to the top of the hour after purchase will be directed to return at a later time to enter his or her voucher number into the drawing. 274Each valid voucher number may only be used one time, for one entry into the drawing. 290CASH4LIFE 291ticket 292number293s cannot be used for entry into the drawing. The odds of winning depend on t309he number of entries received. 314Entry vouchers should not be mailed to the Lottery unless players are contacted by the Florida Lottery and requested to do so.     361(3) Drawing.

    363One drawing will be conducted by the Florida Lottery on March 2, 2018. Winners will be randomly selected from entries submitted and received 386before midnight ET on February 28, 2018. A total of 396120 397valid entries will be drawn by the Florida Lottery using a certified random number generation process. The first through twenty-eighth valid entries drawn will each win a first prize of $1,000. The twenty-ninth through one hundredth valid entries drawn will each win a second prize of $100 in 446Florida Lottery 448449$1,000 A WEEK FOR LIFE455 456Scratch-Off tickets. 458The one hundred first through one hundred twentieth valid entries drawn will 470be used in the order in which they were drawn and in the order of need to select an alternate first prize winner in the event a first prize winner cannot be awarded. Alternate entries will not be dr509awn for second prizes.

    569(5) Notification.

    571(a) The prizewinners in 575the 576second chance drawing will be posted on fl584alottery.com after the drawing.

    588(b) 589The F591lo592r593ida Lottery will attempt to notify each winner by telep603ho604n605e or email using the contact information provided in th615e 616winner’s registration data 619no later than 622one business day 625after the winners are posted on the Florida Lottery’s website. The Florida Lottery deems the winner’s registration data as the winner’s official contact information, including the physical mailing address, and will not attempt to further locate a winner if attempts to reach the winner by telephone or email are unsuccessful.

    6751. 676If the Florida Lottery is unable to have contact by telephone or email with a 691first 692prize winner within two weeks of the date of the posting on the website, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to an alternate winner in accordance with subsection (3), 735above. 736If the Florida Lottery is unable to have personal contact with the alternate winner within two weeks of the date of notification, the alternate winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to the next alternate winner. This process will continue until an alternate winner is contacted or the Florida Lottery has exhausted the list of alternate winners due to unsuccessful notification attempts or due to failure by a winner or an alternate winner to timely provide required claim documentation as set forth in subsection (6), 833below. If the Florida Lottery exhausts the list of alternate winners, 844the prize will not be awarded.

    8502. If the 853Florida Lottery is unable to have contact by telephone or email with a second prize winner within two weeks of the date of the posting on the website, the winner will forfeit his or her right to claim the prize and the prize will not be awarded.

    900(6) How to Claim a Prize. 906To claim any prize in the CASH4LIFE Bonus Bucks Promotion 916drawing, 917the winner must submit to the Florida Lottery the original valid voucher bearing the unique number selected in the drawing. Without such voucher, the winner will forfeit his or her right to claim a prize. 952The voucher should be detached from the CASH4LIFE ticket prior to submission 964to the Florida Lottery 968and the CASH4LIFE ticket should be retained by the player.

    978(a) 979A first prize winner must submit the voucher 987along with a completed Winner Claim Form DOL 173-2, revised 9/13, or Spanish Winner Claim Form DOL 173-2S, revised 9/13 and a copy of acceptable identification as set forth in the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tallahassee, Florida 32399-4011. 1053Forms DOL 173-2 and DOL 173-2S are hereby incorporated by reference 1064and may be obtained at any 1070Florida Lottery retailer or 1074Florida Lottery office, from the Florida Lottery’s website at flalottery.com, or by writing to: Florida Lottery, Customer Service, 250 Marriott Drive, Tallahassee, Florida 32399-4016. The required documentation must be received by the Florida Lottery no later than two weeks after the Florida Lottery has made contact 1120by telephone or email 1124with the winner. If the Florida Lottery has not received the required documentation from a prizewinner by the fourteenth day after notification, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to an alternate winner 1170in accordance with subsections (3) and (5), 1177above. The same timeframe for a winner to return required documentation to the Florida Lottery shall apply to an alternate winner.

    1198(b) A second prize winner must submit the voucher 1207and identification 1209to the Florida Lottery for receipt by the Florida Lottery no later than two weeks after the Florida Lottery has made contact 1231by 1232telephone or email 1235with the winner. 1238If the Florida Lottery has not received the 1246required 1247documentation 1248from a prizewinner by the fourteenth day after notification, the winner will forfeit his or her right to claim the prize and the Florida Lottery will not award the prize1278.

    1279(7) Award of 1282First 1283Prize. Upon 1285the Florida Lottery’s timely receipt of 1291a 1292first 1293prize winner’s required documentation, the Lottery will 1300award 1301the winner 1303a 1304check for 1306$13071,0001309.

    1310(8) Award of Second Prizes. 1315Upon 1316the Florida Lottery’s 1319receipt of 1321a 1322second prize winner’s 1325documentation 1326t1327he Lottery will 1330ship the winner $100 in Florida 1336Lottery 1337“$1,000 A WEEK FOR LIFE” 1343Scratch-Off tickets, except as follows. 1348A winner whose mailing address is outside the state of Florida will receive a check for the value of the Scratch-Off tickets in lieu of the actual tickets. 1376(Value $100)

    1378(91379) Taxes.

    1381(a) 1382Except as specifically described herein, all federal, state and/or local taxes or other fees on the prizes won in the Promotion will be th1406e responsibility of the winner.

    1411(b) 1412Pursuant to applicable provisions of the Internal Revenue Code, 1421federal income taxes 1424will be withheld from 1428a 1429first 1430prize awarded to a nonresident alien claimant 1437at a rate of thirty percent1443. 1444The reporting and subsequent payment of any additional federal, state and/or local taxes shall be the responsibility of the nonresident alien 1465claimant1466.

    1467(1146801469) Oth1471er Restrictions and Provisions.

    1475(a) All prizes are subject to the provisions of Chapter 24, F1487.1488S1489., 1490and rules promulgated thereunder. Prizes shall be paid in accordance with the rule of the Florida Lottery governing payment of prizes. A copy of the current 1516p1517ayment of 1519p1520rizes 1521rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tal1538lahassee, Florida 32399-4011.

    1541(b) All 1543first 1544prize 1545winners will b1548e analyzed for state-owed debt. 1553If a prizewinner is identified as owing an outstanding debt to any state agency or owes child support collected through a court, the debt will be offset in accordance with 1583S1584ection 158524.115(4), 1586F1587.1588S. If the debt is an amount less than the amount of the prize, the balance of the prize after deduction of the debt shall be awarded. If the debt is an amount greater than the amount of the prize, the entire prize amount will be applied to the outstanding debt.

    1639(c) Players must be at least 18 years of age. Persons prohibited by 1652S1653ection 165424.116, 1655F1656.1657S1658., 1659from purchasing a Florida Lottery ticket are not eligible to 1669enter the CASH4LIFE Bonus Bucks Promotion.

    1675(d) By entering the 1679C1680ASH4LIFE 1681Bonus Bucks 1683Promotion, a player gives his or her permission for the Florida Lottery to photograph and/or videotape and record the prizewinner with or without prior notification and to use the name, photograph, videotape, and/or recording of the prizewinner for advertising or publicity purposes without additional compensation.

    1728(e) 1729The 1730CASH4LIFE 1731Bonus Bucks 1733second chance d1736rawing 1737shall be public, held in Tallahassee, Florida, and witnessed by an accountant employed by an independent cer1754tified public accounting firm.
