55-11.005. Admission Eligibility  


Effective on Monday, January 2, 2023
  • 1(1) To be eligible for residence, a veteran must meet the criteria stated in Section 16296.06, F.S., 18and not be determined to be ineligible under Section 27296.07, F.S.

    29(2) The veteran applicant must not be in need of hospitalization or nursing home care and must be ambulatory, substantially able to attend to personal needs, dress, groom, and attend a general dining facility or be in need of extended congregate care.

    71(3) Criteria for admission to the domiciliary home must be consistent with the admission requirements for assisted living facilities set forth in Rule 59A-36.006, F.A.C., and said criteria are incorporated by reference in this rule section.

    107(4) Before admission, each applicant must apply for and receive a certificate of eligibility 121from the VA, using the most current application packet provided by the Florida Department of Veterans’ Affairs that includes any required VA forms and instructions, to explain the supporting documentation that is required154. The application forms 158are not adopted in this rule as permanent links because VA may update its forms; however, the most current VA forms 179can be obtained from any of the following locations:

    284(5) The fully completed Application for Certificate of Eligibility along with all required supporting documentation must be forwarded to the home at the Robert H Jenkins, Jr., Domiciliary Home of Florida, 751 SE Sycamore Terrace, Lake City, Florida 32025. Incomplete applications will be returned to the applicant.

    331(a) The completed application 335with all required supporting documentation 340will be reviewed by an Admissions Committee 347at the Domiciliary 350consisting of representatives of Business Services, Health Services, Social Services, and Food Services.

    363(b) The Admissions Committee will review the application and make a recommendation to the Administrator as to the action to be taken, and whether or not to approve the admission.
