59B-14. Health Plan Consumer Report |
1(1) Each health insurer shall include the following company and contact information when submitting a report required in this section to the Agency for Health Care Administration:
28(a) Name of company;
32(b) Florida company code assigned by the Florida Office of Insurance Regulation;
44(c) NAIC company code as assigned by the National Association of Insurance Commissioners;
57(d) Measurement year;
60(e) Covered lives in Florida on December 31 of the measurement year;
72(f) Year beginning continuous license in Florida;
79(g) Company website;
82(h) Company telephone number;
86(i) Contact name;
89(j) Contact title;
92(k) Contact address;
95(l) Contact direct telephone number;
100(m) Contact e-mail address;
104(n) Contact FAX number;
108(o) Survey vendor name;
112(p) Survey vendor organization;
116(q) Survey vendor telephone number;
121(r) Auditor name;
124(s) Auditor organization;
127(t) Auditor telephone number; and,
132(u) Designate plan type as:
1371. Health plans of health maintenance organizations as defined under Chapter 641, F.S.; or
1512. Health plans of health insurers defined under Chapter 627, F.S.
162(2) The health insurer company contact information shall be reported in a text file as described in subsection 18059B-14.004(8), 181F.A.C., using a tab between each data element.
