61H1-31.006. Reactivation Fee

Effective on Monday, October 28, 2024

1(1) The application fee for reactivation of an inactive status license to active status shall be $250.00; for reactivation of a delinquent status license to active, $250.00. In all cases completion of the requirements of Rule 3761H1-33.006, 38F.A.C., shall be required for reactivation.

44(2) The application fee for reactivation of a retired status license to active status shall be $250.00. Applicants seeking reactivation of a retired license shall comply with the requirements of paragraph 61H1-30.060 (7)(b), F.A.C.
