61H1-31.015. Reinstatement of a Null and Void License Fee

Effective on Monday, January 7, 2013

1Upon approval of the board, reinstatement of a null and void license must be accompanied by an application fee of $250.00. The completion of the requirements of Rule 2961H1-33.006, 30F.A.C. shall be required for reinstatement.
