1The following fees are required for the activities associated with ope12rator examination and licensing.
16(1) Applicants for operator examinations shall submit the following fees with the application. All application fees are non-refundable.

192(3) All 194operators holding a Class 198A, B, 200or 201C 202treatment plant operator 205license shall renew 208their 209license biennially and pay a non-refundable $75 fee to the Department before a renewal license will be issued. 227Operators holding a Class D treatment plant operator license or a 238Level 2391, 2, 3, or 4 water distribution system operator 248license shall renew 251their 252license biennially and pay a non-refundable $50 fee to the Department before a renewal license will be issued.
270(2714272) To obtain a duplicate license or wall certificate, the licensee must submit a written request and pay $25 for each duplicate requested.
295(5296) To reactivate an inactive license, a reactivation fee of $100 shall be paid in addition to the specified biennial license renewal fee.
319(3206321) The fees stated in subsections (1) 328through 329(3303331) above shall be $10 for an application, $10 for 341an 342examination, and $10 for 346a 347license 348or license 350renewal for wards of the state upon submission of written evidence, with the examination 364or license 366application or renewal notice, to the Department that said individual is a ward of the state.
