63M-2.008. Periodic Evaluations  


Effective on Sunday, March 16, 2014
  • 1(1) A periodic evaluation by a Physician, PA or ARNP shall be conducted for youth in a facility who:

    20(a) Have at least one chronic medical condition; or

    29(b) Have a communicable disease; or

    35(c) Are prescribed medications for at least three (3) consecutive months.

    46(2) Periodic evaluations shall be conducted, at a minimum, once every three (3) months.

    60(3) A periodic evaluation is required prior to renewing a prescription for a medication that has expired.

    77(4) Periodic evaluations for pregnant youth 83shall be conducted no less than every four weeks until the eighth month of pregnancy. Pregnant youth shall receive a 103periodic evaluation every two weeks in the eighth month, and weekly thereafter.

    115(5) Each facility shall have a method of scheduling and tracking periodic evaluations.

    128(6) Periodic evaluations conducted on-site shall be documented in the chronological progress notes in the Individual Health Care Record. The documentation shall include subjective findings, objective findings, the medical assessment of the youth, and the plan of care for treatment of the youth.

    171(7) Periodic evaluations conducted off-site shall be documented on the Summary of Off-Site Care 185Form (HS 033), and filed in the 192Individual Health Care Record in the chronological progress notes, in reverse chronological order.
