63N-1.0074. Individualized Mental Health and Substance Abuse Treatment Plans - General Requirements

  • 1(1) An 3Individualized Mental Health Treatment Plan must be recorded on form MHSA 016 or a form developed by the program which contains all the mental health information in form MHSA 016. The Individualized Mental Health/Substance Abuse Treatment Plan Form (MHSA 016, October 2014) is incorporated by reference and is available at 53http://www.flrules.org/Gateway/reference.asp?No=Ref-05368 55or may be obtained by contacting: DJJ, Office of Health Services, 2737 Centerview Drive, Tallahassee, FL 32399. 72The 73Individualized Mental Health Treatment Plan 78must be signed and dated by the 85Mental Health Clinical Staff Person, the treatment team members who participated in development of the plan and the youth.

    104(2) 105An Individualized Substance Abuse Treatment Plan is required when a youth enters on-going substance abuse treatment.

    121(a) Chapter 65D-30, F.A.C., establishes the requirements for individual substance abuse treatment plans provided in facilities licensed under Chapter 397, F.S. or by Service Provider licensed under Chapter 397, F.S.

    151(b) In facilities and programs where substance abuse services are provided by a Licensed Qualified Professional, t168he Individualized 170Substance Abuse Treatment Plan must be developed as follows:

    1791. An 181Individualized Substance Abuse Treatment Plan 186must be 188recorded on the Individualized Mental Health/Substance Abuse Treatment Plan Form (MHSA 016), or a form developed by the program which contains all the substance abuse information in form MHSA 016.

    2182. An 220Individualized Substance Abuse Treatment Plan must be signed and dated by the Substance Abuse Clinical Staff Person, the 238treatment team members who participated in development of the plan and the youth.

    251(3) Development of an 255Individualized Mental Health Treatment Plan, Individualized Substance Abuse Treatment Plan or Integrated Mental Health and Substance Abuse Treatment Plan must include the youth’s parent or legal guardian, unless there documentation of a reason for the parent or legal guardian’s non-involvement in treatment planning.
