63N-1.0101. Mental Health Crisis Assessment

  • 1(1) The superintendent/program director or designee is responsible for consulting the Designated Mental Health Clinician Authority or 18Licensed Mental Health Professional 22who conducts or supervises mental health evaluations at the facility to discuss the youth’s Crisis and associated behaviors.

    40(a) The superintendent/program director or designee and Designated Mental Health Clinician Authority or other 54Licensed Mental Health Professional 58shall confer on those cases viewed as urgent and if it is determined that a mental health emergency exists, the youth shall be transported for emergency mental health services as set forth in Rule 9263N-1.011, 93F.A.C.

    94(b) The superintendent/program director or designee must document consultation with the Designated Mental Health Clinician Authority or other 112Licensed Mental Health Professional 116and referral for Crisis Assessment on form MHSA 014 or a form developed by the program containing all the information required in form MHSA 014.

    141(c) A Crisis Assessment is utilized only when the youth’s Acute Emotional or Psychological Distress or Crisis is 159not 160associated with Suicide Risk Factors or Suicide Risk Behaviors. If the youth’s behavior or statements indicate possible suicide risk, the youth must receive an Assessment of Suicide Risk instead of a Crisis Assessment.

    193(2) The Crisis Assessment must be documented on the Crisis Assessment Form (MHSA 023) or a form developed by the program which contains all the information required in form MHSA 023.     258(a) The Crisis Assessment must include a face-to-face interview of the youth and review of available collateral information. The Crisis Assessment shall provide details of the information obtained by the assessment (i.e., youth statements, behavioral observations, collateral information).

    296(b) The Crisis Assessment must be conducted by a Licensed Mental Health Professional or a non-licensed 312Mental Health Clinical Staff Person 317working under the direct supervision of a Licensed Mental Health Professional.

    328(c) A Crisis Assessment conducted by a non-licensed 336Mental Health Clinical Staff Person 341must be reviewed by a 346Licensed Mental Health Professional 350within 24 hours of the referral.

    356(d) In the circumstance where the Crisis Assessment is conducted by a non-licensed 369Mental Health Clinical Staff Person 374but cannot be reviewed by a 380Licensed Mental Health Professional 384within 24 hours through face-to-face interaction, the 391Licensed Mental Health Professional 395may accomplish a review of the Crisis Assessment within 24 hours of the referral through in-person, telephonic or electronic consultation.
