64B-7.001. Pain Management Clinic Registration Requirements; Exemption Certificates; Inspection and Registration Fees
1(1) Every clinic location, unless exempt under Section 9458.3265(1) 10or 11459.0137(1), F.S., 13must designate a physician and register with the Department on Application to Register Pain Management Clinic, form #DH-MQA 1219 (revised 6/2020), incorporated by reference and available at 40http://www.flrules.org/Gateway/reference.asp?No=Ref-1318842. Clinics claiming an exemption from the registration requirement must apply for a certificate of exemption on Application for Exemption from Pain Management Clinic Registration, form DH-MQA 5031 (6/2020), incorporated by reference and available at 77http://www.flrules.org/Gateway/reference.asp?No=Ref-1318979.
80(2) The clinic’s designated physician must have a full, active, clear and unencumbered license as a medical doctor or osteopathic physician under Chapter 458 or 459, F.S.
107(3) The registration fee shall be $150. The fee for change of physical location or change of ownership shall be $150.
128(4) The annual inspection fee shall be $1,500. Each clinic location will be assessed the fee at the time of inspection regardless of the number of physicians who share the location.
