1Active duty military and veterans seeking licensure pursuant to Section 11456.024(3), F.S., 13shall submit to the Department a completed Florida Department of Health Active Military or Veterans Licensure Application, Form DH5000-MQA (07/2016), incorporated by reference and available at 39https://www.flrules.org/Gateway/reference.asp?No=Ref-0772441. Spouses of active duty military seeking licensure pursuant to Section 52456.024(3), F.S., 54shall submit to the Department a completed Florida Department of Health Active Military Spouse Licensure Application, Form DH5015-MQA (07/2016), incorporated by reference and available at 79https://www.flrules.org/Gateway/reference.asp?No=Ref-0772581.
