64B16-26.203. Pharmacist Licensure by Examination (U.S. Graduates); Application

  • 1Applicants who are at least eighteen (18) years of age and a recipient of a degree from a school or college of pharmacy accredited by an accrediting agency recognized and approved by the United States Department of Education may apply to take the licensure examination.

    46(1) All applications for licensure by examination must be made on board approved form DHMQA 101, Pharmacist Examination Application for U.S. Graduates, 08/2021, which is hereby incorporated by reference, and which can be 79obtained from 81http://www.flrules.org/Gateway/reference.asp?No=Ref-13935, 83the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, 96or the Board’s website at 101http://floridaspharmacy.gov/102. The application must be accompanied with an examination fee and an initial license fee as set forth in Rules 64B16123-26.1001 and 12564B16-26.1002, 126F.A.C.

    127(2) 128In addition to the requirements of subsection (1), t137he applicant must submit proof of completion of an internship program provided by either an accredited school or college of pharmacy or a state board of pharmacy or jointly by both, provided that the program meets the requirements of Rule 17764B16-26.2033, 178F.A.C.

    179(3) An applicant must reapply 184if all requirements for licensure are not met within one (1) year of the receipt of the application.

    202(4) Passing examination scores may be used upon reapplication only if the examination was completed within three (3) years of the reapplication.
