64B16-26.204. Pharmacist Licensure and Pharmacist Technician Registration by Endorsement; Application

  • 1(1)(a) U.S. Graduates. All applications for licensure by endorsement 10for pharmacists who received a degree from a school or college of pharmacy accredited by an accrediting agency recognized and approved by the United States Department of Education 38shall be made on board approved form DOH-MQA100, 08/2021. Pharmacist Licensure by Endorsement Application for U.S. Graduates, which is hereby incorporated by reference, and which can be obtained from 67http://www.flrules.org/Gateway/reference.asp?No=Ref-13936, 69the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, 82or the Board’s website at http://floridaspharmacy.gov/.

    88(b) Foreign Graduates. All applications for licensure by endorsement for pharmacists graduated from a pharmacy program of a school or college of pharmacy located outside of the United States shall be made on board approved form DOH-MQA 1196, 08/2021, Pharmacist Licensure by Endorsement Application for Non-U.S. Graduates, which is hereby incorporated by reference, and which can be obtained from 147http://www.flrules.org/Gateway/reference.asp?No=Ref-13937, 149the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board’s website at http://floridaspharmacy.gov/.

    168(2) 169The application must be accompanied with a non-refundable application fee and initial licensure fee as set forth in Rules 18864B16-26.1001 189and 19064B16-26.1002, 191F.A.C., and in addition, the applicant must submit satisfactory proof of the following:

    204(a) Compliance with the requirements of Sections 211465.007(1)(b)1. 212or 2., and (1)(c), F.S.;

    217(b) Having obtained a passing score on a licensure examination as described in Section 231465.0075(1)(b), F.S.; 233and,

    234(c)1. Having t237wo (2) years of active practice, as defined in Section 247465.0075(1)(c)1., F.S., 249within the immediately preceding five (5) years and have completed 30 hours of 262Board approved continuing education within the two (2) calendar years immediately preceding application. For purposes of this rule only, the Board defines “Board approved continuing education” as any continuing education course offered, sponsored, or approved by the 299Florida Board of Pharmacy, 303the Accreditation Council for Pharmacy Education (310ACPE), or other state board of pharmacy 317which course was accepted by the other state board for licensure or licensure renewal purposes;

    3322. Successful completion of board-approved postgraduate training or a board-approved clinical competency examination within the year immediately preceding application; or

    3523. Successful completion of an internship meeting the requirements of Section 363465.007(1)(c), F.S., 365and Rule 36764B16-26.2033, 368F.A.C., 369within the two (2) years 374immediately preceding application377.

    378(3) All requirements for licensure by endorsement must be met within one (1) year of the receipt of the application. Applicants failing to meet this requirement must reapply.

    406(4) Applicants shall cause the National Association of Boards of Pharmacy, or other similar organization to issue a Transfer of Pharmaceutical Licensure certificate showing examination date, examination results, states of licensure, disciplinary actions, and licensure status.

    442(5) Applicants deemed qualified for licensure by endorsement shall be required to complete the Multistate Pharmacy Jurisprudence Examination – Florida Version. Passing scores on this examination may be used upon reapplication only if the examination was completed within three (3) years of the reapplication.
