1Pursuant to Section 4465.1865, F.S., 6the Board hereby adopts the following list of chronic health conditions for which a pharmacist certified pursuant to Section 25465.1865, F.S., 27can provide specified patient care services to patients of a collaborating physician pursuant to a pending Collaborative Pharmacy Practice Agreement:
47(1) Hyperlipidemia:
49(2) Hypertension;
51(3) Anti-coagulation management;
54(4) Nicotine Dependence;
57(5) Opioid 59use disorder;
61(6) Hepatitis C
64(7) Those chronic health conditions enumerated in Section 72465.1865(1)(b), F.S.
