64B2-12. Fee Schedule |
Effective on Monday, February 28, 2022
1(1) The application fee shall be $100.00.
8(2) The initial license fee shall be $100.00.
16(3) The fee for biennial renewal of an active license shall be $55.00.
29(4) The fee for biennial renewal of an inactive license shall be $4227.50.
43(5) The fee to change licensure status shall be $55.00.
53(6) The fee for reactivation of an inactive license shall be $50.00.
65(7) A delinquent status license shall pay a delinquency fee of $50.00.
