64B20-5.005. Requirements for Renewal of Provisional License

  • 1(1) Renewal is required of a provisional licensee whose license expires prior to completion of all requirements of rule 2064B20-2.001, 21F.A.C.

    22(2) Any person renewing a provisional license to practice speech-language pathology or audiology shall apply to the Department of Health. The application for renewal of provisional license shall be made on Form SPA-5 (rev. 6/2020), entitled “Application for Renewal of a Speech-Language Pathologist or Audiologist Provisional License,” which is incorporated herein by reference, and available from 79http://www.flrules.org/Gateway/reference.asp?No=Ref-12708, 81or the Board of Speech-Language Pathology and Audiology, Department of Health, at http://floridaspeechaudiology.gov/resources/.

    94(3) A provisional license shall be renewed upon receipt in the Board office of a completed application form, SPA-5; completed Form DH-SPA-2A (rev. 6/2020), entitled “Verification of Employment for a Provisional Licensee,” which is incorporated herein by reference, and available from 136http://www.flrules.org/Gateway/reference.asp?No=Ref-12706, 138or the Board of Speech-Language Pathology and Audiology, Department of Health, at http://floridaspeechaudiology.gov/resources/; and the provisional license fee established by rule 15964B20-3.005, 160F.A.C.

    161(4) A provisional license may only be renewed once for a period of three (3) months.
