64B3-6.002. Documentation for Licensure

The following is a list of acceptable documents which shall be submitted to the Board as appropriate for the type of license sought to show eligibility for the license:

(1) Official transcript sent directly from the institution.

(2) A copy of high school diploma.

(3) Copy of a diploma, training certificate or Department of Defense form DD-214 or any other document which verifies pertinent education and experience.

(4) A copy of certificate of attendance or documentation of training required pursuant to Chapters 64B3-3 and 64B3-4, F.A.C., and continuing education required pursuant to Chapter 64B3-11, F.A.C., one (1) hour HIV/AIDS continuing education or affidavit stating that the course will be completed within six (6) months of licensure.

(5) Civil notarized copies of documents of foreign education and translation, if appropriate.

(6) Foreign credentials evaluation which includes a breakdown of all college level courses by credit hours and subject sent directly to the board office by one of the following evaluators:

(a) A regionally accredited U.S. college or university.

(b) American Society for Clinical Pathology Board of Certification.

(c) National Accrediting Agency for Clinical Laboratory Sciences.

(d) Center for Applied Research, Evaluation & Education, Inc.

(e) Foundation for International Services, Inc.

(f) Educational Credential Evaluators, Inc.

(g) International Consultants of Delaware, Inc.

(h) Education Evaluators International, Inc.

(i) International Education Research Foundation, Inc.

(j) Education International.

(k) Josef Silney & Associates International Educational Consultants.

(l) Foreign Academic Credentials Services, Inc.

(m) World Education Services, Inc.

(7) Applicants with a baccalaureate degree in accredited medical technology programs from accredited institutions in the Philippines need only submit official transcripts sent directly by the institution to the Board office. No foreign credentials evaluation is necessary.

(8) Applicants with a degree from an accredited U.S. college or university recognized by the United States Department of Education, or foreign equivalent, whose major is not a chemical or biological science, who otherwise meet the requirements for the license sought pursuant to Chapter 64B3-5, F.A.C., may submit their academic credentials for a determination by the Board that their degree is equivalent to a degree in clinical laboratory, chemical or biological science.

(a) Such determination shall be made by the Board based upon actual degree earned, the number and type of academic science credits earned, and the courses taken. Applicants without the requisite number of academic science credits cannot be determined to have equivalent degrees for the purposes of licensure.

(b) The applicant may be requested by the Board to submit an additional evaluation of the applicant’s academic record from the chairperson of a chemical or biological science department of an accredited U.S. college or university recognized by the United States Department of Education to demonstrate that the applicant’s education is equivalent to a U.S. degree in the chemical or biological sciences which shall be reviewed by the Board in conjunction with the Applicant’s academic credentials.
