64B3-8. Renewal And Reactivation Of Licensure And Inactive Status |
1(1) A clinical laboratory personnel licensee may elect at any time to place the license into an inactive status by filing with the Board a complete form DH-MQA 1229 Florida Department of Health (Department) License Renewal Application (Active and Inactive Status), incorporated by reference at Rule 4764B-9.001, 48F.A.C., and paying the inactive status fee of Rule 5764B3-9.006, 58F.A.C. For the purpose of this section, a complete application shall be a renewal form provided by the Department on which the licensee affirmatively elects inactive status.
85(2) An inactive status licensee may change to active status at any time provided the licensee meets the following continuing education. For licenses that have been inactive for one (1) year or less, the licensee must obtain twelve (12) hours of board approved continuing education. For licenses that have been inactive for one (1) year and (1) day or longer, the licensee shall obtain twenty-four (24) hours of board approved continuing education. The licensee shall be requested to provide copies of all continuing education hours; and
171(a) Pay the active status fee of Rule 17964B3-9.004, 180F.A.C.; and,
182(b) Pay the reactivation fee of Rule 18964B3-9.005, 190F.A.C.; and,
192(c) Pay, if applicable, the change of status fee of Rule 20364B3-9.010, 204F.A.C.
