64B5-15.019. Renewal Fees for General Anesthesia, Pediatric Moderate Sedation, and Parenteral Moderate Sedation Permits

Effective on Tuesday, September 12, 2017

1(1) The biennial renewal fee for a General Anesthesia Permit shall be $200.00.

14(2) The biennial renewal fee for a 21Moderate 22Sedation Permit shall be $200.00.

27(3) The biennial renewal fee for a Pediatric 35Moderate 36Sedation Permit shall be $200.00.

41(4) Renewal fees are not refundable.
