64B5-2.0142. Application for Health Access Dental License

Effective on Tuesday, August 31, 2021

1Any person wishing to be issued a Health Access Dental License shall apply to the Board of Dentistry. The application shall be made on the Application for Health Access Dental License form #DH-MQA 1154 (Rev. 08/2020), available at 39http://www.flrules.org/Gateway/reference.asp?No=Ref-13425, 41hereby adopted and incorporated by reference, and can be obtained 51at http://www.floridadentistry.gov.
