64B6-3.001. Application for Initial License

  • 1(1) The Department shall issue a license to each applicant who has completed the appropriate form, paid the initial licensure fee, and whom the Board certifies has met the licensing requirements of Chapter 484, Part II, F.S., and minimal procedures and equipment requirements of Chapter 64B6-6, F.A.C. The application shall be made on the Hearing Aid Specialist Application for Licensure Form, Form DH-MQA 1156 (revised 7/2023), hereby adopted and incorporated by reference, and can be obtained from 78http://www.flrules.org/Gateway/reference.asp?No=Ref-15949 80or the 82Board of Hearing Aid Specialists’ website at 89http://floridashearingaidspecialists.gov/applications/app-initial-licensure.pdf90.

    91(2) Payment of the fee and all other licensing requirements required by this rule shall be met within ninety (90) days of notification of licensure eligibility, or the eligibility certification becomes null and void and the person must reapply for licensure.
