64B7-30.006. Notice of Noncompliance

Effective on Sunday, December 31, 2017

1Pursuant to Sections 4120.695 5and 6456.073(3), F.S., 8the Board designates the following as minor violations for which a notice of noncompliance may be issued for the first violation thereof:

30(1) Failure to provide written notice of licensee’s current mailing address and place of practice in violation of Rule 64B7-28.015, F.A.C.

51(2) Failure to have proof of active insurance available at an establishment as required by subsection 6764B7-26.003(4), 68F.A.C.

69(3) Failure to include license number or numbers in any advertisement as required by Section 84480.0465, F.S.
