64B8-51.007. Fees for Licensure Applications, Initial Licensure, and Inspection

Effective on Thursday, March 31, 2016

1(1) License application fee is $100.

7(2) License application fee for facility license is $100.

16(3) Initial licensure fee is $100 and a $5 special fee to fund efforts to combat unlicensed practice.

34(4) Inspection fee for facility is $100 biennially.
