64B9-3.002. Qualifications for Examination

  • 1An applicant seeking certification to take the licensure examination shall submit 12a completed Nursing Licensure by Examination Application, form number DH-MQA 1094, 04/22, 24hereby incorporated by reference, and may be obtained from 33https://www.flrules.org/Gateway/refere34nce.asp?No=Ref-14784, 36or 37Nursing Licensure by Re-Examination Application, form number DH-MQA 1120, 04/22, 47hereby incorporated by reference, and may be obtained from 56http://www.flrules.org/Gateway/reference.asp?No=Ref-14785, 58demonstrating 59that he or she meets the qualifications prescribed by the Nurse Practice Act, Chapter 464, F.S. 75These forms are also available from the Board’s website84: http://floridasnursing.gov86. The demonstration shall include:

    91(1) A high school diploma, or a high school diploma equivalent.

    102(2) For graduates of an approved nursing program, a notice of graduation or of completion of the requirements for graduation. For graduates of an approved program equivalent, an official transcript or equivalent documentation which identifies all courses completed with a minimum acceptable passing score established by the institution or program at which each course was completed that meet graduation requirements. For graduates of programs in a country other than the United States, the applicant must obtain a report by a credentialing agency that meets the requirements of Rule 19064B9-3.014, 191F.A.C.

    192(3) For an applicant writing the examination for practical nurses on the basis of practical nursing education equivalency, 210a completed Practical Nurse Equivalence (PNEQ) Application Letter 218or an official certified transcript which 224sets forth graduation from an approved professional program. 232The form is available from the Board office or on the Board’s website: http:floridasnursing.gov.

    246(4) If the applicant is a graduate of a foreign prelicensure education program not taught in English, or if English is not the applicant’s native language, successful completion of one of the following:

    279(a) A minimum score of 540 on the paper version; 207 on the computerized version or 76 on the internet based version, on the Test of English as a Foreign Language (TOEFL) Examination;

    312(b) A minimum overall score of 55 with a minimum speaking score of 55 on the Michigan English Test (MET);

    332(c) A minimum score of 6.5 overall with a 7.0 on the spoken portion of the academic version of International English Language Testing System (IELTS), or

    358(d) A minimum score of 300 on the Occupational English Test (OET).
