64B9-7. Fees |
1The following fees are prescribed by the Board.
9(1) For application to sit for the examination as provided in section 21464.008, F.S.23:
24(a) For registered nurse and licensed practical nurse fifty dollars ($50.00).
35(b) Applicants for the NCLEX-CAT examination must pay the fee for sitting for the examination directly to the examination vendor.
55(2) For application for licensure without examination by endorsement as provided in section 68464.009, F.S.70:
71For registered nurse and licensed practical nurse fifty dollars ($50.00).
81(3) Initial license as a registered nurse or a licensed practical nurse fifty dollars ($50.00).
96(4) For application for APRN certification as provided in section 106464.012, F.S., 108one hundred dollars ($100.00).
112(5) For APRN registration as a dispensing practitioner pursuant to section 123465.0276, F.S., 125one hundred dollars ($100.00).
129(6) For renewal of RN or LPN license as provided in section 141464.013, F.S., 143sixty-five dollars ($65.00).
146(7) For renewal of an APRN license, fifty dollars ($50.00).
156(8) Pursuant to section 1601009.66(6), F.S., 162the Department shall collect a five dollar ($5.00) fee upon initial licensure or renewal of all LPN, RN, and APRN licenses for the Student Loan Trust Fund.
189(9) For application to change from active to inactive status as provided in section 203464.014, F.S.205:
206(a) For registered nurse fifty five dollars ($55.00).
214(b) For licensed practical nurse fifty five dollars ($55.00).
223(c) For APRN license fifty dollars ($50.00).
230(10) For renewal of an inactive license as provided in section 241464.014, F.S.243:
244(a) For registered nurse fifty five dollars ($55.00).
252(b) For licensed practical nurse fifty-five dollars ($55.00).
260(c) For APRN license fifty dollars ($50.00).
267(11) For reactivation or change of status of an inactive or delinquent license, as provided in sections 284456.036(8) 285and 286464.014, F.S.288:
289(a) For registered nurse fifty-five dollars ($55.00).
296(b) For licensed practical nurse fifty-five dollars ($55.00).
304(c) For APRN license fifty dollars ($50.00).
311(12) A delinquent status licensee shall pay a delinquency fee of fifty-five dollars ($55.00) when the licensee applies for inactive status or for reactivation.
335(13) The inactive status biennial renewal fee shall be fifty-five dollars ($55.00).
347(14) The continuing education provider application and renewal fees shall each be two hundred fifty dollars ($250.00).
364(15) The application fee for approval of a nursing program shall be one thousand dollars ($1,000.00).
381(16) The initial retired status license fee shall be fifty dollars ($50.00).
393(17) The duplicate license fee shall be twenty-five dollars ($25.00).
