65G-5.012. Documentation of Services.

Effective on Wednesday, January 18, 1995

1The provider shall maintain documentation that the supports and services for which billings or invoices are submitted have been provided. For providers of supported living coaching and personal assistance this shall include written documentation of the dates, times and summary of support provided during contact with the individual.
