69K-17.0030. Direct Disposer/Establishment; Fees

Effective on Monday, February 18, 2019

1(1) The direct disposer nonrefundable application fee shall be $375.

11(2) The direct disposer renewal fee shall be $375.

20(3) The direct disposal establishment nonrefundable application fee shall be $450.

31(4) 32A member of the United States Armed Forces, such member’s spouse, and a veteran of the United States Armed Forces who separated from service within 2 years preceding the application for licensure are exempt from the individual application fee. The military record must show such member is currently in good standing or such veteran was honorably discharged.
