69O-191.095. Change of Address

Effective on Thursday, May 28, 1992

1Every licensed HMO shall notify the Office of Insurance Regulation, Bureau of Specialty Insurers, in writing, 60 days prior to such change, of any change in its Florida principal or corporate business address. Any health service clinics or centers owned or controlled by the HMO shall maintain current addresses with the corporate office and shall be made available upon request by the Office.
