59A-6.026. Records and Reports

  • 1The center shall maintain records on all consumers who have been tested. A system shall be established to insure the prompt location of each tested individual’s record.

    28(1) As a minimum, the center shall maintain a record for a minimum of two years from the date of testing which shall contain the following items of information:

    57(a) A sequence number.

    61(b) Name.

    63(c) Address.

    65(d) Date of Birth.

    69(e) Sex.

    71(f) Laboratory test taken, including the date and time the specimen was collected and the date the test was returned, completed.

    92(g) Laboratory used.

    95(h) Tests performed and the identity of the individual performing the test for all tests performed in the center.

    114(i) Date test results were provided to the consumer.

    123(2) A copy of all test results and reports shall be maintained for a period of no less than two years.
