5F-8. Permitting And Inspection Requirements For Amusement Devices And Amusement Attractions |
1(1) The owner of an amusement ride shall make written application to the department for an annual permit for each amusement ride in accordance with Section 27616.242(5), F.S., 29on department form DACS 03432, Amusement Ride Annual Permit Application.
39(2) The current Affidavit of Compliance required by Section 48616.242(4), F.S., 50shall be submitted with the written application for an annual permit on department form DACS 03433, Affidavit of Compliance and Nondestructive Testing.
72(3) The current Affidavit of Nondestructive Testing required by Section 82616.242(6)(a), F.S., 84shall be submitted with the written application for an annual permit on department form DACS 03433, Affidavit of Compliance and Nondestructive Testing, unless the amusement ride is exempt pursuant to Section 115616.242(6)(d), F.S.
117(4) The request for inspection required by Section 125616.242(5)(b)6., F.S., 127shall be submitted on department form DACS 03429, Request for Inspection or Reinspection, with the written application for an annual permit.
148(5) Each amusement ride permitted in this state shall have a USAID number assigned and shall have a USAID plate affixed to the amusement ride by the owner. If a USAID number has been previously assigned to the amusement ride and there is no USAID plate affixed to the ride, the inspector will assign a new USAID number to the ride, the owner shall affix the plate to the ride and the department shall assess a fee for replacement of the lost USAID plate as provided by Rule 2365F-8.012, 237F.A.C.
