64B1-7.004. Notice to the Agency of Mailing Address and Place of Practice of Licensee
Effective on Sunday, November 3, 2019
1(1) It shall be the duty of each licensee to provide written notification to the Department of the licensee’s current mailing address and place of practice. For purposes of this rule, “place of practice” shall mean the address of the primary physical location where the certificateholder practices acupuncture.
49(2) Any time that the current mailing address or place of practice of any licensee changes, written notification of the change shall be provided to the Department within 10 days of the change. Written notice should be sent to the following address: Board of Acupuncture, 4052 Bald Cypress Way, Bin C-06, Tallahassee, Florida 32399-3256. Electronic notification to MQA.Acupuncture@flhealth.gov, is acceptable, however, it is the responsibility of the licensee to ensure that the electronic notification was received by the Department.
