64B15-12.009. Osteopathic Faculty Certificate  

Effective on Tuesday, September 15, 2020
  • 1(1) An Osteopathic Faculty Certificate may be issued by the Department to a faculty member of a school accredited by the American Osteopathic Association upon the request of the dean of the school if the faculty member has demonstrated to the Board that:

    44(a) The faculty member is currently licensed to practice osteopathic medicine in another jurisdiction of the United States; and,

    63(b) Is a graduate of a school of osteopathic medicine accredited by the American Osteopathic Association; and,

    80(c) Files an application on 85board approved application form, DH-MQA 1193 (Revised 5/20), Application for Osteopathic Medical Faculty Certificate, 99which is hereby incorporated by reference, and may be obtained from 110http://www.flrules.org/Gateway/reference.asp?No=Ref-12130, 112from the Board of Osteopathic Medicine, 4052 Bald Cypress Way, Bin #C06, Tallahassee, Florida 32399-3256 or by web at www.doh.state.fl.us/mqa/osteopath/index.html, and 133otherwise meets the requirements contained in Section 140459.0055, F.S.; 142and,

    143(d) Has submitted the application fee required by subsection 15264B15-10.002(4), 153F.A.C.

    154(2) An Osteopathic Faculty Certificate authorizes the holder to practice only in conjunction with his or her teaching duties at an accredited school of osteopathic medicine or in its affiliated teaching hospitals or clinics.

    188(3) Faculty Certificates shall automatically expire upon termination of the holder’s relationship with the school or after a period of 24 months, whichever occurs first. Faculty Certificates are subject to cancellation or revocation by the Board for failure to comply with Chapters 456 and 459, F.S., and rule Division 64B15, F.A.C.
