64E-28.004. Registration Requirements for a Guest Tattoo Artist

  • 1(1) A guest tattoo artist seeking registration by the department shall:

    12(a) Submit, at least fourteen (14) days prior to pra22c23ti24c25ing tattooing in any li30c31ensed tattoo establishment or li36c37ensed temporary tattoo establishment, a 42c43ompleted appli45c46ation for registration to the department on Form DH 4150, 7/12, Application for Guest Tattoo Artist Registration, which is incorporated herein by reference and which can be obtained from the environmental health section of the county health department, which has jurisdiction for the tattooing program in the county where the tattoo artist lives, or from the internet at 104https://www.flrules.org/Gateway/reference.asp?No=Ref-01525 106or 107http://www.myfloridaeh.com/community 108/Tattoo/index.html. An appli111c112ation shall be 115c116onsidered 117c118omplete if all of the information requested on the form is provided, along with the fee spe135c136ified in Rule 13964E-28.011, 140F.A.C., and the following do145c146umentation is atta149c150hed:

    1511. A 153c154opy of a government issued photo identifi161c162ation 163c164onfirming the appli167c168ant is at least 18 years of age.

    1762. A copy of an a182c183tive li185c186ense, registration, or 189c190ertifi191c192ation in another jurisdi196c197tion.

    1983. A copy of the certificate of training proving completion of a course on blood-borne pathogens and communicable diseases, which meets the requirements specified in paragraph (b).

    225(b) Complete a department approved 230c231ourse on blood-borne pathogens and 236c237ommuni238c239able diseases with having a244c245hieved a minimum score of seventy percent (70%) on the course examination.

    2571. Completion of the educational course mentioned in paragraph (b), shall not be required when a tattoo artist has completed a blood-borne pathogens and communicable diseases course as a requirement for license, registration, or certification in a jurisdiction outside of the state and, as determined by the department, the course requirements met or exceeded those prescribed in Sections 315381.00775(2)(b)4.and 3165., F.S., and Rule 32064E-28.006, 321F.A.C.

    3222. A tattoo artist may submit a course curriculum to the department and request the department conduct a review for determination of the requirements specified in subparagraph (1)(b)1. Requests for curriculum review shall be submitted prior to submission of a completed application for registration of a guest artist.

    370(2) A guest tattoo artist’s registration is valid for up to fourteen (14) 383c384onse385c386utive days, throughout the State of Florida.

    393(3) A guest tattoo artist shall not perform tattooing without an a405c406tive guest tattoo artist registration.

    411(4) A guest tattoo artist registration shall not be transferrable from one person to another.
