64E-28. Tattooing |
1(1) Licensure of a permanent tattoo establishment:
8(a) Prior to performing tattooing, a 14c15ompleted appli17c18ation shall be submitted to the county health department on form DH 4151, 8/12, Application for Tattoo Establishment Licensure, which is incorporated herein by reference and which can be obtained from the environmental health section of the county health department, which has jurisdiction for the tattooing program in the county where the establishment is physically located, or from the internet at 79https://www.flrules.org/Gateway/reference.asp?No=Ref-01526 81or http://www.myfloridaeh.com/community/Tattoo/index.html. An appli85c86ation shall be 89c90onsidered 91c92omplete if all of the information requested on the form is provided, along with the fee spe109c110ified in Rule 11364E-28.011, 114F.A.C.
115(b) A li118c119ense for a tattoo establishment is valid for one year from the date it is issued.
135(c) A li138c139ense whi141c142h has not been renewed on or before the date of expiration shall be deemed inactive. If this day falls on a weekend or holiday, a tattoo artist li171c172ense must be renewed on or before the first working day after the date of expiration.
188(2) Licensure of a temporary tattoo establishment:
195(a) A completed appli199c200ation to be submitted to the county health department on form DH 4151 at least thirty (30) days prior to performing tattooing. An appli224c225ation shall be 228c229onsidered 230c231omplete if all of the information requested on the form is provided, along with the fee spe248c249ified in Rule 25264E-28.011, 253F.A.C.
254(b) A li257c258ense for a temporary tattoo establishment is valid for up to fourteen (14) 271c272onse273c274utive days in 277c278onjun279c280tion with a 283c284onvention or similar event for which the license is issued.
294(3) No tattooing shall be performed at an establishment that does not have an active license.
310(4) A tattoo establishment license shall not be transferrable from one location or person to another.
