64J-2. Trauma |
1Each hospital approved as a trauma center shall be issued a DH Form 2032-Z, 15January 20171180, 19Level I Trauma Center Certificate of Approval, DH Form 2043-Z, 29January 20311320, 33Level II Trauma Center Certificate of Approval, or DH Form 1721-Z, 44January 20461470, 48Pediatric Trauma Center Certificate of Approval, which are incorporated by reference and available from the department, 64as defined by subsection 6469J-70271.00172(73474), F.A.C76. The certificates shall include:
81(1) The date effective and the date of termination;
90(2) The hospital’s name; and
95(3) The approved trauma center level.
