64B13-10.001Application for Certification
NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 196, October 8, 2013 issue of the Florida Administrative Register.
The change is in response to comments submitted by the Joint Administrative Procedures Committee in a letter dated October 24, 2013. The changes are as follows:
64B13-10.001, first paragraph shall read correctly as “To be certified to administer and prescribe topical ocular pharmaceutical agents a licensed practitioner must submit a completed application, DH-MQA 1128 (12/2013) "Application for Licensure As Certified Optometrist," hereby incorporated by reference, provided by the Board; remit the application fee for certification specified in subsection 64B13-6.001(7), F.A.C.; and demonstrate compliance with the following requirements:”
A technical change will be done to correct the “Law Implemented” to read correctly as “463.0055, 463.006 FS.”
The changes to Form DH-MQA 1128 are:
The requirement for a photograph has been removed from the instructions of the application form.
Page two of the application: the requirement for a photograph has been removed.
Page two of the application: the requirement for a place of birth has been removed.
Page two of the application: language has been added to clarify if an email address is provided, it is subject to
disclosure as a public record.
