64B14-4.003: Documentation of Eligibility for Licensure

NOTICE IS HEREBY GIVEN THAT on October 7, 2008, the Board of Orthotists and Prosthetists, received a petition for Douglas A. Taylor, CP. Pursuant to Rule 64B14-4.003, F.A.C., Petitioner seeks a variance and waiver of the provisions of subsection 64B14-4.003(1)(b), F.A.C. Specifically, the Petitioner requests that subsection 64B14-4.003(1)(b), F.A.C., be waived in regards to the educational course work which was completed before CAAHEP’s implementation.

This Petition will be considered by the Board at its meeting on November 7, 2008, 9:00 a.m.
