    PURPOSE AND EFFECT: The purpose of this rule development is to continue revision of existing screening and assessment forms, including a Screening Form that is separate from the current practice of Form 701A, Department of Elder Affairs Prioritization Form, September 2008, and Form 701T, Department of Elder Affairs CARES Mini Assessment, to bring these forms up to date.
