    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 47 No. 183, September 21, 2021 issue of the Florida Administrative Register.

    The Board received written comments from the Joint Administrative Procedures Committee on Rule 64B6-2.003.  Additionally, the Board made changes to the proposed form set forth in the proposed rule. 

     

    The changes to the form and revision dates are as follows:

     

    64B6-2.003 Licensure by Examination.

    Any person desiring to be licensed as a hearing aid specialist shall apply to the Department at least one hundred twenty (120) days prior to the date the examination is to be administered, and by completing the Hearing Aid Specialist Application for Examination on Form DH-MQA 1155 (Revised 12/20217/2021) hereby adopted and incorporated by reference, which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-            , the Board of Hearing Aid Specialists’ website at http://floridashearingaidspecialists.gov/applications/app-licensure-examination.pdf.  As a condition of licensure by examination, the following shall be completed;

    (1)    through (3) No Change.

    On Page 5, Section I NBC-HIS Board Certification or Passed the ILE requirements for eligibility no longer requires that the 12 months of active practice must occur within the previous 18 months and provide proof of active practice by submission of two contracts per month within the previous 18 months.

     

    On Page 5, Section III Licensed in another state without NBC-HIS Board Certification requirements for eligibility no longer requires that 12 months of active practice by out-of-state licensees must occur within the previous 18 months and they provide proof of active practice by submission of two contracts per month within the previous 18 months.
