RULE NO.:RULE TITLE:
64B8-51.001Manner of Application
NOTICE IS HEREBY GIVEN that on January 24, 2024, the Board of Medicine, received a petition for variance or waiver filed by Roberta Graham, CPE, CME, CCE, CLHRP, regarding Rule 64B8-51.001(2), F.A.C., which requires that all applications must include an official transcript from a school of electrology which identifies the credits taken by home study or correspondence courses and those taken in classroom settings. Petitioner request that the Board accepts the certificate of completion of 315 hours of electrolysis training from Pittsburgh Beauty Academy, earned in 1994, to meet the requirement and intent of the rule and that same be accepted in lieu of providing the official transcript. Pittsburgh Beauty Academy is no longer in business and the location which held the school suffered a fire in their basement in early 2000, when digital copies of these records were not available, leading to the loss of transcripts and records predating that year. Comments on this petition should be filed with the Board of Medicine, 4052 Bald Cypress Way, Bin #C03, Tallahassee, Florida 32399-3053, within 14 days of publication of this notice.
A copy of the Petition for Variance or Waiver may be obtained by contacting: Paul Vazquez, J.D., Executive Director, Board of Medicine/MQA, Paul.Vazquez@flhealth.gov.
