59G-4.210. Visual Care Services

Effective on Wednesday, June 29, 2016

1(1) This rule applies to 6any person or entity prescribing or reviewing a request for visual care services and to 21all providers of visual care services who are enrolled in or registered with the Florida Medicaid program.

38(2) All 40persons or entities described in subsection (1) 47must be in compliance with the provisions of the Florida Medicaid Visual Care Services Coverage 62Policy, June 2016, incorporated by reference.
