5F-8.0142. Reporting of Accidents and Mechanical, Structural or Electrical Defects (Transferred)

Effective on Monday, May 30, 2005

1(1) Accidents shall be reported to the department in accordance with Section 13616.242(14), F.S. 15Accidents shall be reported in writing on department form DACS 03428, Written Accident Report.

29(2) Mechanical, Structural or Electrical defects shall be reported to the department, in writing, in accordance with Section 47616.242(14), F.S., 49on department form DACS 03430, Mechanical, Structural or Electrical Defect Report.
