60FF-1. State Communications Definitions; Usage Qualifications; Exemptions And Clearances |
1(1) 2All of the following formal submittals to the Department shall comply with the standards of Rule 1860FF-1.004, 19F.A.C.:
20(a) 21Notices of Security Concern;
25(b) 26Parts I and II of Exemption Requests;
33(c) 34Clearance Requests;
36(d) 37Network Solution Replacement Declarations.
41(2) 42Customers shall use one of the following means of making submittals:
53(a) 54Through the provisions of the CSAB System or;
62(b) 63Via electronic mail with attachments to SUNCOMRequests@dms.myflorida.com with the title of the submittal and the name of the Customer in the Subject line. Note that if the request contains sensitive information, use of electronic mail may pose security risks.

125(3) 126The Customer shall provide the following standard information with all submittal packages.
138(a) 139SUNCOM account number;
142(b) 143The Customer account number;
147(c) 148Customer organization name, address, city, state, zip code;
156(d) 157The submittal author’s name and 162contact information;
164(e) 165The name and contact information of the person who is an employee of the Customer holding a full-time position who shall speak on behalf of the Customer and shall be available to answer related questions.
200(f) 201Category of service the submittal pertains to (e.g., Voice, Data, Conferencing, Wireless).
213(4) 214Submittals shall use common practices of readability including tables of contents where appropriate, headings, executive summaries or cover letters, proper grammar and spelling. Recommended examples shall be provided through the Portfolio of Services as they become available.
251(5) 252Single submittals that describe the same conditions in multiple locations or describe conditions that are repeated in multiple events over time shall be accepted by the Department in lieu of multiple submittals if all of the locations, events and timing of the events are named in the submittal.
300(6) 301The Department will protect any information contained in these submittals in accordance with exemptions to Chapter 119, F.S.
