63M-2.0037. Verification of Credentials

Effective on Sunday, March 16, 2014

1(1) The facility Superintendent, Program Director or designee are responsible for verification of credentials prior to contract execution for all health care providers at that facility.

27(2) A copy of the following documentation shall be maintained in the health care provider’s service agreement file at the facility and at the respective regional office:
54(a) Current license;

57(b) Resume; and

60(c) Current Basic, Advanced, or Pediatric Advanced Cardiac Life Support Certification that includes training on the automated external defibrillator.
