1(1) Application forms:
4(a) All applicants for licensure as a Clinical Laboratory Personnel Director shall apply to the Department on Form #DH-MQA 3008 (10/2020) “Clinical Laboratory Personnel Initial & Upgrade Licensure ‒ 33Director,” 35http://www.flrules.org/Gateway/reference.asp?No=Ref-12886, 37which is incorporated by reference herein.
43(b) All applicants for licensure as a Clinical Laboratory Personnel Supervisor shall apply to the Department on Form #DH-MQA 3009 (10/2020), “Clinical Laboratory Personnel Initial & Upgrade Licensure 71‒ Supervisor,” 74http://www.flrules.org/Gateway/reference.asp?No=Ref-12814, 76which is incorporated by reference herein.
82(c) All applicants for licensure as a Clinical Laboratory Person92nel Technologist shall apply to the Department on Form #DH-MQA 3011 (10/2020), “Clinical Laboratory Initial & Upgrade Licensure ‒ Technologist,” 113http://www.flrules.org/Gateway/referen114ce.asp?No=Ref-12816, 116which is incorporated by reference herein.

159(e) Any licensee requesting to add a specialty to his or her current license shall apply to the Department on Form #DH-MQA 3012 (10/2020), “Clinical Laboratory Licensure 186‒ Adding Specialty,” 190http://www.flrules.org/Gateway/reference.asp?No=Ref-12817, 192which is incorporated by reference herein.
198(2) Copies of all application forms can be obtained from t209he Board office at 2134052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257 222or from the web at http://www.floridasclinicallabs.gov/licensing. The application must be accompanied by the appropriate application fee required by Rules 24164B3-9.001 242and 24364B3-9.002, 244F.A.C.
245(3) After one year from the date of the original submission of an application, a new application and fee shall be required from any applicant who desires to be considered for licensure.
277(4) With regard to persons who have been issued a social security number by the Federal Government, disclosure of a social security number is mandatory pursuant to Section 305456.013(1)(a), F.S., 307and social security numbers are used to allow efficient screening of applicants and licensees by a Title IV-D child support agency to assure compliance with child support obligations.
