1(1) A licensee shall have each teletherapy unit and gamma stereotactic radiosurgery unit fully inspected and serviced during source replacement or at least every 5 years, whichever comes first, to assure proper functioning of the source exposure mechanism.
39(2) This inspection and servicing shall only be performed by persons specifically licensed to do so by the department, an agreement state, or the U.S. Nuclear Regulatory Commission.
67(3) A licensee shall maintain a record of the inspection and servicing for the duration of the license. The record shall contain:
89(a) The inspector’s name;
93(b) The inspector’s radioactive materials license number;
100(c) The date of inspection;
105(d) The manufacturer’s name and model number and serial number for both the treatment unit and source;
122(e) A list of components inspected;
128(f) A list of components serviced and the type of service;
139(g) A list of components replaced; and
146(h) The signature of the inspector.
