64B10-11. Licensure |
1Any person desiring to be licensed as a nursing home administrator shall apply to the Board of Nursing Home Administrators 21Application fees, pursuant to Rule 64B10-12.001, F.A.C., shall apply.
30(1) For licensure by examination, 35the application shall be made on the Application for Nursing Home Administrators by Examination, form DH-MQA-NHA002, (06/20), hereby adopted and incorporated by reference, which can be obtained from 63http://www.flrules.org/Gateway/reference.asp?No=Ref-12185, 65or 66the Board’s website at 70http://floridasnursinghomeadmin.gov/applications/application-licensure-examination-nha.pdf71. Licensure eligibility requirements are listed in subsection 7964B10-11.002(2), 80F.A.C.
81(2) For licensure by endorsement, the application shall be made on the Application for Nursing Home Administrators by Endorsement, form DH-MQA-NHA5936, (6/2020), hereby adopted and incorporated by reference, which can be obtained from 114http://www.flrules.org/Gateway/reference.asp?No=Ref-12186, 116or the Board’s website at http://floridanursinghomeadmin.gov/applications/application-licensure-endorsement-nha.pdf. Licensure eligibility requirements are listed in subsection 12964B10-11.002(3), 130F.A.C.
