64E-5.350. Reports of Transactions Involving Nationally Tracked Sources

  • 1Each licensee who manufactures, transfers, receives, disassembles, or disposes of a nationally tracked source shall complete and submit to the NRC a National Source Tracking Transaction Report as specified in subsections (1) through (5) of this section for each type of transaction.

    43(1) Each licensee who manufactures a nationally tracked source shall complete and submit a National Source Tracking Transaction Report. The report must include the following information:

    69(a) The name, address, and license number of the reporting licensee;

    80(b) The name of the individual preparing the report;

    89(c) The manufacturer, model, and serial number of the source;

    99(d) The radioactive material in the source;

    106(e) The initial source strength in becquerels (curies) at the time of manufacture; and

    120(f) The manufacture date of the source.

    127(2) Each licensee that transfers a nationally tracked source to another person shall complete and submit a National Source Tracking Transaction Report. The report must include the following information:

    156(a) The name, address, and license number of the reporting licensee;

    167(b) The name of the individual preparing the report;

    176(c) The name and license number of the recipient facility and the shipping address;

    190(d) The manufacturer, model, and serial number of the source or, if not available, other information to uniquely identify the source;

    211(e) The radioactive material in the source;

    218(f) The initial or current source strength in becquerels (curies);

    228(g) The date for which the source strength is reported;

    238(h) The shipping date;

    242(i) The estimated arrival date; and

    248(j) For nationally tracked sources transferred as waste under a Uniform Low-Level Radioactive Waste Manifest, the waste manifest number and the container identification of the container with the nationally tracked source.

    279(3) Each licensee that receives a nationally tracked source shall complete and submit a National Source Tracking Transaction Report. The report must include the following information:

    305(a) The name, address, and license number of the reporting licensee;

    316(b) The name of the individual preparing the report;

    325(c) The name, address, and license number of the person that provided the source;

    339(d) The manufacturer, model, and serial number of the source or, if not available, other information to uniquely identify the source;

    360(e) The radioactive material in the source;

    367(f) The initial or current source strength in becquerels (curies);

    377(g) The date for which the source strength is reported;

    387(h) The date of receipt; and

    393(i) For material received under a Uniform Low-Level Radioactive Waste Manifest, the waste manifest number and the container identification with the nationally tracked source.

    417(4) Each licensee that disassembles a nationally tracked source shall complete and submit a National Source Tracking Transaction Report. The report must include the following information:

    443(a) The name, address, and license number of the reporting licensee;

    454(b) The name of the individual preparing the report;

    463(c) The manufacturer, model, and serial number of the source or, if not available, other information to uniquely identify the source;

    484(d) The radioactive material in the source;

    491(e) The initial or current source strength in becquerels (curies);

    501(f) The date for which the source strength is reported; and

    512(g) The disassemble date of the source.

    519(5) Each licensee who disposes of a nationally tracked source shall complete and submit a National Source Tracking Transaction Report. The report must include the following information:

    546(a) The name, address, and license number of the reporting licensee;

    557(b) The name of the individual preparing the report;

    566(c) The waste manifest number;

    571(d) The container identification with the nationally tracked source.

    580(e) The date of disposal; and

    586(f) The method of disposal.

    591(6) The National Source Tracking Transaction Report discussed in subsections (1) through (5) of this section must be submitted to the NRC by the close of the next business day after the transaction. A single report may be submitted for multiple sources and transactions. The reports must be submitted to the National Source Tracking System by using:

    648(a) The on-line National Source Tracking System;

    655(b) Electronically using a computer-readable format;

    661(c) By facsimile;

    664(d) By mail to the address on the NRC Form 748 National Source Tracking Transaction Report Form; or

    682(e) By telephone with followup by facsimile or mail.

    691(7)(a) Each licensee shall correct any error in previously filed reports or file a new report for any missed transaction within 5 business days of the discovery of the error or missed transaction. Such errors may be detected by a variety of methods such as administrative reviews or by physical inventories required by regulation.

    745(b) In addition, every year each licensee shall reconcile the inventory of nationally tracked sources possessed by the licensee against that licensee's data in the National Source Tracking System. The reconciliation must be conducted during the month of January in each year. The reconciliation process must include resolving any discrepancies between the National Source Tracking System and the actual inventory by filing the reports identified by subsections (1) through (5) of this section.  In order to reconcile each transaction, the licensee shall file a report for missed transactions or file a corrected report for previously submitted reports containing inaccuracies. By January 31 of each year, each licensee must submit to the National Source Tracking System confirmation that the data in the National Source Tracking System is correct.

    872(8) Each licensee that possesses Category 1 nationally tracked sources shall report its initial inventory of Category 1 nationally tracked sources to the National Source Tracking System by January 31, 2009 or as specified in 90710 C.F.R. section 20.2207(h), 9111-1-13 edition, which is herein incorporated by reference and is 921available at 923http://www.flrules.org/Gateway/reference.asp?No=Ref-03456 925or 926http://www.gpo.gov/fdsys/pkg/CFR-2013-title10-vol1/pdf/CFR-2013-title10-vol1-sec20-2207.pdf927. Each licensee that possesses Category 2 nationally tracked sources shall report its initial inventory of Category 2 nationally tracked sources to the National Source Tracking System by January 31, 2009 or as specified in 96210 C.F.R. section 20.2207(h), 9661-1-13 edition968. The information may be submitted by using any of the methods identified by paragraphs (6)(a) through (6)(e) of this section. The initial inventory report must include the following information:

    998(a) The name, address, and license number of the reporting licensee;

    1009(b) The name of the individual preparing the report;

    1018(c) The manufacturer, model, and serial number of each nationally tracked source or, if not available, other information to uniquely identify the source;

    1041(d) The radioactive material in the sealed source;

    1049(e) The initial or current source strength in becquerels (curies); and

    1060(f) The date for which the source strength is reported.
