    64B6-2.003Licensure by Examination

    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 91, May 8, 2020 issue of the Florida Administrative Register in response to written comments from the Joint Administrative Procedures Committee on Rule 64B6-2.003.  Additionally, the Board made changes to the proposed form set forth in the proposed rule. 

     

    The changes to the form and revision dates are as follows:

     

    64B6-2.003 Licensure by Examination.  Any person desiring to be licensed as a hearing aid specialist shall apply to the Department at least one hundred twenty (120) days prior to the date the examination is to be administered.

    (1) The Board, or its designee, shall certify for examination each applicant who:

    (a) Has completed the Hearing Aid Specialists Application for Examination, Form DH-MQA 1155 (Revised 7/202010/19), hereby adopted and incorporated by reference, which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-      , the Board of Hearing Aid Specialists’ website at http://floridashearingaidspecialists.gov/applications/app-licensure-examination.pdf;

    (b) through (f) No change.

    (2) through (3) No change.

    Additionally, the Board made changes to the proposed form set forth in the proposed rule. 

    64B6-2.003(1)(a):  Form DH-MQA 1155, which is incorporated by reference (which also includes DH-MQA 1159) has removed requirements of affirmations by the applicant.

    The application sets forth possible penalties that may be imposed pursuant to s. 456.072 F.S.  The form cites chapter 64B6-8 rather than “Rule 64B6-8.”  The form and the rule now refer to Rule 64B6-8.003(3), F.A.C.
