1A person making claim for the $5,000 reward provided for by Section 14790.164, F.S., 16shall utilize the Claim of Reward Form, FDLE-OGC Form #1, rev. January 2001 and incorporated by reference and the Law Enforcement Endorsement Form, FDLE-OGC Form #2, rev. January 2001 and incorporated by reference, both of which can be obtained from the arresting agency, and the Prosecutor’s Endorsement Form, FDLE-OGC Form #3, rev. January 2001 and incorporated by reference, which can be obtained from the prosecuting office. FDLE has samples of these forms which may be requested from the Department of Law Enforcement, Office of General Counsel, P. O. Box 1489, Tallahassee, Florida 32302-1489.
